We are excited to announce that starting April 2024, our Family Nurse Partnership and Health Visiting services will be transitioning from Black Country Healthcare NHS Foundation Trust to Shropshire Community Health NHS Trust. This change is part of the commitment to continuously enhance the care and support available to families in Dudley.
For current and future service users, this means you will continue to receive the high-quality care you expect, just under a different NHS Trust. To ensure a smooth transition and maintain the continuity of care, we have worked closely with Shropshire Community Health NHS Trust.
What does this mean for you?
- Uninterrupted Services: There will be no disruption in the care you and your family receive.
- New Contact Information: While your care team may remain the same, how you contact us will change slightly.
- Enhanced Support: With this transition, you will be able to access a wider range of services and support.
For more information on the services and how to access them under Shropshire Community Health NHS Trust, please visit their service pages:
We thank you for your trust and patience during this transition. Our goal is to make this change as seamless as possible for all involved, ensuring that your family's health and wellbeing continue to be our top priority.
