OneWolverhampton is a collaboration of health, social care, voluntary, and community organisations working together for better health and care.

By adopting a population health approach, they aim to improve the health and wellbeing of the people of Wolverhampton, focusing on both physical and mental health to help reduce health inequalities across the city.

They work across organisations and with local communities to better understand their unique needs, using data and evidence to guide their actions and focus on changes that will make a real difference to people’s lives.

The partnership includes:

  • Primary Care Networks (six across the City)
